First introduced in the United States and in place for more than 30 years, it supports older adults with complex needs who may otherwise be at risk of long-term care placement.

The Program of All-Inclusive Care for the Elderly (PACE) is an established community-based approach that brings health care, home supports, wellness, and social connection together for older adults, so care feels coordinated instead of fragmented.
PACE Canada is adapting this model for Canada by working with existing public funding and alongside existing providers and local community supports, helping communities deliver more connected care with clear coordination and measurable outcomes.

Why This Model, Why Now?
Canada’s health and social care systems weren’t designed for today’s aging population. Many older adults want to stay at home, but services are often fragmented and hard to navigate. Caregivers are overwhelmed and frequently end up coordinating care without support. At the same time, hospitals and long-term care are under extreme capacity pressure.
PACE Canada helps close these gaps by bringing integration, accountability, and person-centred support to older adults and their families , so care becomes easier to access, easier to coordinate, and easier to adjust over time.
