Education & Resources
If you support older adults and the system feels fragmented, you are not alone. PACE is designed to make care more connected and easier to coordinate across teams and services.
PACE Research and Evidence
PACE is backed by strong evidence. In the United States, studies consistently show that PACE can reduce hospitalizations, help older adults remain at home longer by delaying long-term care placement, and achieve high satisfaction among participants and caregivers. Over time, outcomes also indicate that the model can be cost-effective by preventing avoidable crises and better coordinating care. In Ontario, the Burlington Community Wellness Hub pilot demonstrated measurable results and has since expanded to multiple hub locations, reinforcing the potential for scalable, community-based delivery.
Health is not just about treatment — it is about the systems that shape daily life. This video examines the social determinants of health in Alberta and the growing movement toward wellness-based models of care.
